The ACT Plan: Accountability, Competition, Transparency
President Trump has put forth several innovative policy proposals and has asked the private sector to come to the table with their own. American innovators and manufacturers responded, committing over $300 billion in domestic medical R&D, manufacturing, and workforce investments since January 2025. This policy blueprint serves as a means to achieve President Trump’s vision and policies, and do so without the need for harmful government price controls that would lead to adverse impacts for patients and the U.S. economy.
The plan also includes a blueprint for meaningful health insurer reform. Insurance conglomerates have been gaming the system and taking advantage of their vertical integration to increase profits at the expense of patients. MFP outlines specific policies that will fix the broken system, hold insurers accountable, and lower costs for everyday Americans.
Our roadmap to deliver President Trump’s healthcare vision:
DIRECT TO CONSUMER PURCHASING OF MEDICINES: Allow patients and families to bypass costly middlemen fees and mark-ups to buy medicines directly through drugmakers directly or via pharmacy of their choice.
END FOREIGN FREE RIDING: Require foreign countries to pay their fair share for brand drugs through a NATO-style agreement with contribution minimums as a percent of GDP.
ADDRESS ECONOMIC AND NATIONAL SECURITY THREATS POSED BY CHINA AND OTHERS: Increase protections for U.S. intellectual property and patents and ensure fair access to markets for American products and services.
ACCELERATE INVESTMENT IN AMERICAN R&D, MANUFACTURING, AND WORKERS: Incentivize expanded investments and reduce domestic barriers to further increase U.S. medical innovation, drug development, manufacturing, and jobs.
REFORM SECRETIVE INSURER-PBM & HOSPITAL RX PRICING PRACTICES: Require insurer-PBMs and hospitals to pass all drug rebates and discounts directly to patients, families, and those most in need. No more exemptions from federal anti-kickback laws.
ESTABLISH GREATER HEALTHCARE PRICE TRANSPARENCY: Apply accountability and transparency measures to health plans, medical treatments and services so that patients, doctors, and employers can respond to visible price signals.
ELIMINATE WASTE, FRAUD, AND ABUSE THAT HARMS PATIENTS AND TAXPAYERS: Reform healthcare entitlement programs in ways that improve patient benefits while eliminating waste fraud, and abuse and fix the regulatory approval process to be faster and less expensive.
LEVERAGE AI TO ADVANCE AMERICA’S GLOBAL LEADERSHIP IN MEDICAL INNOVATION: Develop public private AI incubators at NIH, FDA, and HHS to drive R&D and economic growth, commercialize new treatments, and save costs.

FAMILIES & Individuals
Provide Plan Options, Not Insurer Profit Centers - Shift premium ACA credits from direct insurer payment to allowing consumers to choose plan of choice (ACA, HSA, others)
Cash Price Deductibility - Allow patients and families to shop for care and pay cash if cheaper (cash pay applies to deductibles)
Enhance HSA Flexibility - Broader personal use, portability, and tax treatment for all HSA plans
Make Catastrophic Plans Available Nationally – Remove obstacles to catastrophic plan availability in all 50 states
Small businesses & workers
Deregulate ACA Small Business Health Plan Market - Treat small business plans same as all other employer-based plans
Reform Small Biz Health Care Tax Credit - Reestablish health plan coverage credit by ending requirement to buy an ACA plan
Expand AHPs and CHOICE Act Arrangements - Broaden AHPs to collectively negotiate lower prices for small businesses
Rural Health Plan Exemptions - Exempt plans and non-profit cooperatives from onerous state/federal insurance regs.
HEAlth plan & provider freedom
Make Private Medical Practice Great Again – ACA/federal payment policies led to mass consolidation (MDs employed via insurers, hospital systems, etc.). Adopt payment reforms to place direct patient care on same level as other models
Pre-empt Certificate of Need Laws - Address state Certificate of Need laws allowing incumbent hospitals to veto competition, restricting supply and resulting in less choice and higher prices
Insurance Competition Across State Lines - Let insurance plans compete for across state lines
